Welcome to the Parenthood.com "Baby Weight" Survey!

Baby Weight SurveyTell us about your experiences with losing your "baby weight" and you'll be entered in a drawing for a $100 VISA Gift Card!
 
The purpose of this survey is to learn about new mom’s experiences with losing “baby weight” (weight gain during pregnancy) after delivery of your first child. You may still take this survey if you have more than one child.

Please do not complete this survey if you have not given birth in the past 5 years. Please do not complete this survey if you are younger than 18 years old.

This survey has 30 questions and should take no more than 15 to 20 minutes to complete.

Data from this survey may be used for a research study on postpartum weight loss. Data may be published in a research article, but all respondents will remain anonymous. Information provided by this survey will remain confidential. There is no way that any answer you give can be identified as coming from you. Anonymous results will also be shared with the Parenthood.com.

You may skip any question you do not feel comfortable answering, or do not know the answer to. Even if you decide to stop the survey early, or do not completely answer all questions, you may still enter to win a $100 Visa gift card. Instructions for entering to win the $100 Visa Gift Card can be found at the end of the survey.

TERMS & CONDITIONS: Instructions for entering to win the $100 Visa Gift Card can be found at the end of the survey. Your entry must be received no later than 12:00 pm (EST), March 15, 2009. Winner is chosen at random from all submissions and will be notified by email. Tne winner will be chosen at random and notified via email on Monday, March 16th. The winner will be given until midnight March 18, 2009 to respond to the notification email. If the winner does not respond by that time, a second email address will be selected at random, and an email notification will be sent to the winner on March 19, 2009.
