SURVEY: Children's Eating Habits

Is your child a great eater who loves everything? Does she have strong food preferences? We're conducting a study on kids' (age 3-6) eating habits and food choices and we want to hear from you!

This survey is designed for parents who are 18 years or older, and who have at least one child between the ages of 3 and 6 years old. If this applies to you, please fill out our survey as completely and accurately as possible.

This survey has 25 questions and should take no more than 5 minutes to complete.

TERMS & CONDITIONS: Instructions for entering to win the $100 Visa Gift Card can be found at the end of the survey. Your entry must be received no later than 12:00 pm (EST), March 3, 2011. Winner is chosen at random from all submissions and will be notified by email. Tne winner will be chosen at random and notified via email on March 4, 2011. The winner will be given until midnight March 11, 2011 to respond to the notification email. If the winner does not respond by that time, a second email address will be selected at random, and an email notification will be sent to the winner on March 12, 2011.
